Domácí zápasy hraje na Městském fotbalovém stadionu Miroslava Valenty, který má kapacitu 8 121 diváků. Klubové barvy jsou tmavě modrá a bílá. Klub vznikl 1. července 2000 jako 1. FC Synot sloučením FC SYNOT (dříve TJ Jiskra Staré Město) a FC SYNOT Slovácká Slavia Uherské Hradiště.
Po korupční aféře v českém fotbale v sezóně 2003/04 klub opustila firma Synot a klub se přejmenoval na 1. FC Slovácko. Kvůli finančním problémům sestoupil v sezóně 2006/07 z první ligy. Ihned po sestupu, v červnu 2007, klub znovu změnil majitele. V roce 2009 klub koupil licenci na 1. ligu od Čáslavi a po dvou sezónách se tak znovu vrátil mezi českou fotbalovou elitu.
Mezi nejznámější hráče se řadí: Milan Kerbr, Miroslav Kadlec, Jiří Kowalík, Miroslav Valenta st., Zdeněk Jánoš, Michal Kadlec, Lubomír Vlk.
